What Do We Do?

CVHN considers it a privilege to work with members of our community who provide healthcare for uninsured and underinsured kids in our communities….

School Nurse and other school personnel – A sick child is identified by a teacher, counselor or principal who notifies the school nurse. The school nurse obtains parental permission and faxes a simple referral form to CVHN indicating the problem.

CVHN Case Manager – The case manager uses information on the referral form to determine what type of healthcare appointment the child needs. The case manager contacts the appropriate healthcare provider and schedules an appointment. She then notifies the family as to the schedule and determines if the family needs assistance with transportation.

Community Volunteer - If assistance with transportation to an appointment is needed, a pre-screened community volunteer picks up the parent and child and takes them to the doctor. If medication is needed, the volunteer can stop by the pharmacy. The volunteer then returns the child either to home or school.

Health Care Provider – CVHN has dentists, doctors, mental health professionals, imaging companies and medical laboratories in its network who volunteer a certain number of visits per month. Most often, a child is seen quickly and is back in class or home in a reasonable time. The healthcare provider sends a report to the CVHN case manager, who then follows up and schedules any additional healthcare the child needs.
